In August 1976, Westinghouse shipped several large electric rotors aboard the SS LESLIE LYKES owned by Lykes Brothers Steamship Co. The vessel contained six cargo holds with various compartments. In the No. 3 lower tween deck (LTD), cotton bales were stowed alongside drill pipe secured by chains and turnbuckles. The manhole access to this compartment was covered by bags of flour. On August 31, while sailing in rough seas, crew members heard a clanking noise. Approximately twelve hours later, smoke was detected coming from the No. 3 hold. The crew could not access the compartment through the manhole because it was blocked by cargo. They attempted to extinguish the fire by injecting CO2 and later steam. When the vessel reached El Ferrol, Spain, firefighters cut a hole in the bulkhead between holds to access the fire. During firefighting operations, both holds were flooded with seawater, damaging Westinghouse’s cargo. Investigation revealed a broken turnbuckle on one of the chains securing the pipe stow.
