NMS owned a multi-purpose tank barge (NMS 1950) designed and manufactured by St. Louis Ship. The barge had a design defect causing it to trim aft instead of forward during unloading operations. To correct this problem, St. Louis Ship suggested installing a sea valve in the rake compartment that would allow flooding to adjust the barge’s trim. NMS implemented this solution and operated the barge with this procedure for over three years. In August 1979, the barge was loaded with sulfuric acid owned by Allied Chemical and destined for Kaiser Aluminum’s dock. During unloading operations supervised by a PSC tankerman, the rake compartment was not flooded as required. When the tankerman left the barge to obtain a replacement hose after a leak was discovered, the barge developed a severe aft list and eventually sank.
