On June 17, 1993, Adelaide Andrews visited a radiologist’s office in Santa Monica, California, where she provided personal information including her Social Security number on a patient form. The office receptionist, Andrea Andrews, copied this information and subsequently moved to Las Vegas where she attempted to open credit accounts using Andrews’ Social Security number with her own last name and address. TRW received four credit report requests matching Andrews’ Social Security number, last name, and first initial, and disclosed Andrews’ credit history on July 25, 1994, September 27, 1994, October 28, 1994, and January 3, 1995. Andrews did not discover these improper disclosures until May 31, 1995, when she sought to refinance her mortgage. She filed suit on October 21, 1996, more than two years after the first two disclosures but within two years of discovering the violations.
